Extension Request

Use this form if you will not graduate within the published length of your degree or certificate program and you need financial aid for the last few quarters OR if you are asking to receive continued funding for a change of program.

Petition for Reinstatement after Time Away

Use this form if your aid was suspended at the end of your last quarter of attendance and you are returning after an absence of one quarter or more.

Enrollment Revision Request

Use this form if you are changing your enrollment status and an adjustment to your financial aid is needed.  NOTE:  You can only request an adjustment to your financial aid once per quarter; all changes are final.

Program of Study

Use this form if you have been asked by our office to confirm your program of study at YVCC.
