If you are not close enough to utilize the college's testing labs it is your responsibility
to locate an eligible proctor. Proctors must be approved by the eLearning office at least
one week before the due date of the test. |
Fill out each field, print and have your prospective proctor sign the form. The form can
be returned either by scanning it into your computer and sending via email or use the
Fax number on the form, or mail. Please read the form carefully and follow all directions
to avoid delays in your ability to take your test on time. |
